How do I make an appointment?

Can I get in today?

Do I need a referral?

Pearl doesn't require one to be seen. Some insurance plans do, usually HMO plans, some Medicare Advantage plans, Kaiser, Tricare Prime and VA coverage. Please check with your plan before your visit. If your plan requires one, send it to us before your visit: have your doctor's office fax it to (877) 456-6788, or call or text us at (503) 284-2000 and we'll help you get it to us.

Which doctor will I see?

New patients are seen by Dr. Zac Egidi, a board-certified foot and ankle surgeon, or Dr. Grace Yang, a board-certified, fellowship-trained podiatrist. You can read about them at Dr. Egidi and Dr. Yang.

Do you take walk-ins?

Before and during your first visit

What should I do before my first visit?

Two things before your first appointment: complete the new-patient paperwork using the link we text you, and text us a photo of your insurance card (front and back) and your photo ID at (503) 284-2000. That lets us have your chart and insurance on file, so check-in takes a minute. Didn't get the forms link? Text us and we'll resend it. Please arrive 15 minutes early, and bring any outside X-rays or MRI images on a CD if you have them.

What happens at a new-patient visit?

Your provider takes a history, examines your feet and ankles, and may take X-rays in the office if needed. You'll leave with a diagnosis, a treatment plan, and clear next steps, whether that's home care, a procedure, orthotics, or a follow-up.

Do you treat children?

Yes, and we love it. Kids get extra care and patience at Pearl, and our doctors treat pediatric sports injuries with one goal: getting your child back to the sport they love. We also perform ingrown toenail procedures on children and teens regularly, and we know how to make that visit calm. We understand that choosing a doctor for your child is a big decision, and we hope to make it an easier one. For patients under 18, we ask that a parent or legal guardian come along to every visit.

Insurance and payment

Do you take my insurance?

We're in-network with more than 200 plans, including Regence BCBS of Oregon, Providence, Moda Health, UnitedHealthcare, Cigna, Aetna, PacificSource, Medicare and Medicare Advantage, and we see workers' compensation patients, including SAIF and Majoris. We are not currently accepting patients with the Oregon Health Plan or other Medicaid plans as their primary coverage. Not sure whether your plan is one of ours? We can confirm whether we're contracted with your plan, but we aren't able to see your individual benefits, so please check your deductible, copay and coinsurance with your plan before your visit.

Do you take the Oregon Health Plan (OHP) or Medicaid?

We're not currently accepting patients whose primary coverage is the Oregon Health Plan or another Medicaid plan or CCO (including Health Share, CareOregon, Trillium and PacificSource Community Solutions). If OHP is your secondary coverage behind Medicare or a commercial plan, we can see you. Your CCO's member services line can help you find a podiatrist who is accepting OHP patients.

Do you see workers' compensation or car accident injuries?

Yes. We treat work-related and motor vehicle accident injuries, including SAIF and Majoris workers' compensation claims. Have your claim number, date of injury, employer or carrier, and adjuster's contact information ready when you call; we confirm authorization with the carrier before your visit is scheduled.

How much will my visit cost?

If you're using insurance, we're not able to tell you in advance. Every plan is different, and we can't see your individual plan's deductible, copay, coinsurance or how much of your out-of-pocket maximum you've met, so we can't estimate what your visit will cost. Your plan can: call the member services number on your card, or check your plan's online portal, before your visit. Your copay is collected at check-in; anything your plan leaves to you after it processes the claim is billed to you.

A quick guide to the terms your plan will use:

Deductible: the amount you pay each year before your plan starts paying. Until it's met, you pay the plan's contracted rate for the visit yourself.

Copay: a flat amount (for example $40) you pay for a specialist visit, usually collected at check-in. Some plans use a copay instead of the deductible for office visits; others apply both.

Coinsurance: after the deductible is met, the percentage of the contracted rate you still pay (for example 20%) while the plan pays the rest.

Out-of-pocket maximum: the most you'll pay in a plan year for covered care. Once you reach it, the plan pays 100% of covered services for the rest of the year.

Procedures, X-rays, injections and orthotics are often applied to your deductible rather than your copay, so a visit that includes a procedure can cost more than the copay alone.

Without insurance, we offer a cash-pay discount: discounted rates for payment in full at the time of your visit. With the discount, a new-patient visit is $250, or $300 with X-rays, and follow-ups are $125, or $175 with X-rays. Procedures are priced separately and your provider tells you the price before anything is done; for example, an ultrasound-guided cortisone injection is $350 per site, and our Ingrown Toenail Package is $425, which includes the office evaluation, permanent correction of one nail border, local anesthetic, dressing, and your first follow-up visit (a second toe is $175 more). Ask us for the full cash-pay price list. We accept Visa, Mastercard, American Express, Discover, HSA and FSA cards, and CareCredit.

I don't have insurance. Can I still be seen?

Yes. We see self-pay patients every day and offer a cash-pay discount for payment in full at the time of service: $250 for a new-patient visit or $300 with X-rays, $125 for a follow-up, and package pricing for common procedures such as ingrown toenail correction ($425) and heel-pain injections ($350). You'll receive an itemized receipt, and you can request a written estimate before your visit.

What if my plan is out of network?

You can still be seen. Some plans pay part of an out-of-network visit and some pay nothing, so please call your plan and ask whether you have out-of-network benefits for podiatry and what your out-of-network deductible is. Many out-of-network patients choose our cash-pay discount instead, with rates set in advance and due at the time of service; we'll give you an itemized receipt you can submit to your plan for any reimbursement it offers.

How do I pay a bill?

Balances after insurance are billed to you by statement and through a secure text link. You can pay online through InstaMed (the link on your statement), through the payment link we text you, by phone at (503) 284-2000, or in the office. Questions about a balance? Treatments and services

What conditions do you treat?

Heel pain and plantar fasciitis, bunions, hammertoes, ingrown toenails, toenail fungus, warts, corns and calluses, flat feet, arthritis and gout, neuromas, ankle sprains, sports injuries, fractures and trauma, tendon problems, and pediatric foot concerns. We offer both non-surgical care (injections, custom orthotics, bracing, and PRP therapy in the office) and surgery, from ingrown-nail correction to bunion, hammertoe, ligament and tendon repair, fusion, and total ankle replacement.

Do you treat diabetic foot wounds?

No. Pearl doesn't treat diabetic wounds or ulcers. Please contact your primary care doctor or a wound care center; if the wound is infected or you have a fever, go to the emergency room.

Do you make custom orthotics, and will my insurance cover them?

Yes. They start with a visit with one of our doctors, who evaluates your feet and writes the prescription; you then meet with our pedorthist for a mold of your foot. Our orthotics are manufactured in Bend, Oregon, and we pride ourselves on offering the best orthotics experience in Portland. These are truly customized to you, including fitting them into your footwear (ski boots and cycling shoes included). We offer a cash-pay discount of $500 per pair. If you'd like us to bill your insurance, a $165 deposit is collected before the fitting appointment. Coverage for custom orthotics varies widely and we're not able to check your DME benefits for you, so please call the number on your card before your fitting and ask whether code L3000 is covered, whether prior authorization is needed, and what your deductible and coinsurance are. Whatever your plan doesn't pay is your responsibility. Ask us for our orthotics insurance guide with the full list of questions.

Do you do surgery, and where?

Yes. Our surgeons perform foot and ankle surgery, including bunion, hammertoe, tendon, ligament, fusion and total ankle replacement procedures. Surgery is typically performed at our partner facility, OATH Surgery Center, an outpatient center that is a lower-cost alternative to hospital surgery, or at a hospital when your case calls for it. Surgery is scheduled after a consultation with your surgeon.

How much does ingrown toenail treatment cost?

With insurance, your plan's copay, deductible and coinsurance apply, and only your plan can tell you what that adds up to. Without insurance, we offer a cash-pay discount: our Ingrown Toenail Package is $425 and includes the office evaluation, permanent correction of one nail border (matrixectomy), local anesthetic, dressing, and your first follow-up visit. A second toe or border is $175 more. Most ingrown toenails are treated the same day as your evaluation.

Can I get a second opinion, or send my X-rays ahead?

Yes to both. Book a new-patient visit and bring your imaging on a CD, or have the imaging center fax the report to (877) 456-6788 and call or text us at (503) 284-2000 to let us know it's coming. We'll review it with you at your visit.

Policies

What is your cancellation policy?

Life happens, and we're always happy to reschedule. We just ask for at least 24 hours' notice so we can offer your time to another patient who's waiting; call or text us at (503) 284-2000 and we'll find you a new time. Appointments cancelled with less than 24 hours' notice are charged a $100 late-cancellation fee, and missed appointments are charged a $100 no-show fee.

What if I'm running late?

Please arrive 15 minutes early, and on time at minimum. If you arrive more than 15 minutes late, we may not be able to see you and the visit is treated as a missed appointment, with the $100 fee. Running behind? Call or text us at (503) 284-2000 and we'll tell you whether we can still fit you in.

How do I get my records, a form filled out, or a refill?

Call or text us at (503) 284-2000 and we'll take care of it. For records, we'll send you a release form to sign. For forms such as FMLA, disability or work notes, drop them off at the office or call or text us to arrange it. For refills, ask your pharmacy to send the request to us, or call or text us.

After hours

What if I need help after hours or on a weekend?

Our office is open Monday through Friday, 8 AM to 5 PM. If you have a medical emergency, call 911 or go to the nearest emergency room. For anything else, call or text us at (503) 284-2000 and our team will get back to you first thing the next business morning.

I had surgery with you recently and something's wrong.

Call or text us right away at (503) 284-2000 during office hours. After hours, go to the emergency room or call 911 for fever, spreading redness, bleeding through the dressing, a cast or splint that feels too tight, or new numbness; for anything else, leave a message and we'll call you first thing next business morning.
